South 啵啵直播秀 Urban Indian Health

South 啵啵直播秀 Urban Indian Health is a non-profit corporation that operates two Federally Qualified Health Centers and Patient Centered Medical Homes, providing medical, behavioral health, and cultural services in Sioux Falls and Pierre. Through this project, SDUIH is engaging in strategic conversations for planning the integration of a pharmacist within the health care team at SDUIH sites. SDUIH鈥檚 mission is: 鈥渢o provide total quality medical care for Native American people and the economically disadvantaged residing in Urban areas of South 啵啵直播秀. Care provided is not limited to just the medical needs but to provide holistic quality care for the entire family. Physical and mental healthcare are essential for a healthy society.鈥

Avera Health Plans serves more than 88,000 members through cost-effective and innovative health plans. Founded in 1999, Avera Health Plans provides coverage for individuals and families with out access to employer insurance or who need a Medicare supplement policy, as well as to employers looking to provide quality and affordable health benefits. Avera Health Plans is closely integrated into this project, assisting with the development of payer models that benefit both patients and payers. Avera Health Plans has a 鈥渄edication to provide care and services to everyone who needs it.鈥

Horizon Health Care provides quality patient-centered care to patients in Howard, SD and the surrounding regions. Horizon is a Community Health Center and a Medicaid Health Home. Through the 1815 project, a Lewis Drug pharmacist has been integrated into the health care team at Horizon, allowing Horizon to better serve patients and provide key hands-on information on models of pharmacists working within the ambulatory care workflow. Horizon Health Care鈥檚 mission is to provide 鈥渞ural communities with access to high-quality, affordable primary healthcare services.鈥

Community Health Center of the Black Hills

The Community Health Center of the Black Hills is a health care provider based in Rapid City, SD and serving the Black Hills region. It is a Federally Qualified Health Center serving uninsured or underinsured patients. The Community Health Center of the Black Hills has begun working with the 1815 Project team to implement Medication Therapy Management (MTM) models and provide key feedback to develop those models to maintain to increase quality of care and sustainability. The Community Health Center of the Black Hills鈥檚 mission is 鈥渢o provide quality medical, dental, and mental health care to the community.鈥

SDSHSP is a key partner in supporting South 啵啵直播秀 pharmacists and this project, as well as promoting trainings. Its mission is 鈥渢o provide leadership and education to support its members in helping people make the best use of medications.鈥

Avera 

Avera Health is an integrated health system based in Sioux Falls, South 啵啵直播秀. Avera has six regional centers in Mitchell, Pierre, Sioux Falls, and Yankton, South 啵啵直播秀 and Marshall, Minnesota and works through 37 hospitals, 215 primary and specialty care clinics, and 40 senior living facilities. Avera鈥檚 innovative efforts toward integrating pharmacists in healthcare teams and help developing sustainable MTM models for both patients and health systems are key to this project. Avera is also a leader in Telehealth. Avera鈥檚 mission is 鈥渢o make a positive impact in the lives and health of persons and communities by providing quality services guided by Christian values.鈥

Lewis is a family-owned neighborhood store and pharmacy with 57 locations in South 啵啵直播秀, Iowa, and Minnesota. Lewis has provided quality pharmacy services to the region for 80 years, leading the way with key services including Smart Sync and SmartPack. Lewis is a key partner in developing the Medication Therapy Management (MTM) model through the implementation by and communication with Lewis pharmacists, as well as with developing patient and clinical outcomes integral to the development of the project.

Haisch Pharmacy is a community pharmacy located in Canton, SD that accepts most major prescription and insurance plans. Haisch Pharmacy is a collaborating pharmacy with the 1815 Project and is a key partner in developing and practicing Medication Therapy Management (MTM) models.

In 2016, 啵啵直播秀 Care was purchased by Avera Health, allowing it to continue to provide quality insurance options.

American Pharmacists Association

The American Pharmacists Association is the largest association of pharmacists in the nation, providing a variety of resources and leadership for pharmacists, pharmaceutical scientists, student pharmacists, and pharmacy technicians. APhA has been a key partner by providing training to 14 team members and partners of the 1815 project to provide Medication Therapy Management (MTM) and Diabetes training to other pharmacists in the South 啵啵直播秀 region. APhA鈥檚 mission is to 鈥渓ead the profession and equip members for their role as the medication expert in team-based, patient-centered care.鈥

Center for Family Medicine

Center for Family Medicine 

As a family medicine clinic, the Center for Family Medicine is committed to utilizing multiple modalities through their well-established primary care approach. This allows CFM to treat patients for all of their health care needs, including acute and chronic disease management, prenatal care, and substance use. In the area of substance use, CFM focuses on medication-assisted treatment (MAT). MAT is most effective when a patient commits to change, and their primary care focuses on "whole-person" health. This model addresses all patient needs by combining traditional medicine and behavioral therapy. CFM's team consists of physicians, therapists, a pharmacist, and a nurse case manager. They also collaborate with local behavioral health organizations to ensure patient needs are met. Its mission: 鈥淭he Center for Family Medicine is devoted to providing compassionate, patient-centered care for the entire family in an educational setting. We are committed to improving the health of our patients, their families, and our communities.鈥

Project Recovery

Project Recovery

Project Recovery is the leading provider of telehealth SUD services throughout the state. The center is located in Rapid City, SD, and led by Dr. Steven Tamang, who has been a champion for MAT services throughout the state. PR was formed to address the gap in access to MAT providers throughout the state which remains a key issue. They currently have nine providers with a DATA 2000 waiver and are taking on new patients. PR will provide telehealth services to patients enrolled in the START-SD program as well as provide technical expertise and mentorship to local provider.

Brookings Behavioral Health and Wellness

Brookings Behavioral Health and Wellness

 Brookings Behavioral Health and Wellness is a privately owned, non-profit 501(c)(3) community mental health center and community substance abuse treatment center accredited by the State Division of Behavioral Health. BBHW serves all the state funded patients in Brookings County and provides a wide variety of counseling, prevention education and case management services. BBHW is led by their CEO, Mary Beth Fishback and has established themselves as a leader addressing mental health in the community. This has included being a key member of the Brookings Mental Health Coalition and Brookings Area Mental Health Task Force. The staff at BBHW includes a Physician Assistant, licensed clinicians, LPN and multiple Case Managers. BBHW鈥檚 mission is 鈥淭o provide comprehensive, integrated behavioral health services that promote well-being and quality of life for all.鈥

Lutheran Social Services of South 啵啵直播秀

Lutheran Social Services

LSS of South 啵啵直播秀 is a faith-based organization working to respond to changing needs in South 啵啵直播秀 communities. Their mission is 鈥淚nspired by God's love, we care for, support and strengthen individuals, families and communities.鈥 LSS touches the lives of South 啵啵直播秀ns 45,781 times each year with a variety of programs and services statewide. LSS is one of the largest private non-profit human service agencies in South 啵啵直播秀 and they have offices in each of the target communities. LSS serves people of all ages, faiths, races and economic status with professional, confidential and affordable services. LSS is committed to hiring a Care Coordinator to assist patients and families impacted by OUD to receive the needed medical and social services.

Face It Together

Face it TOGETHER 

Face-It TOGETHER is a nationally-focused addiction wellness nonprofit that provides professional peer coaching to those impacted by addiction, including loved ones. Their mission is to get people with addiction and their loved ones well. By pairing members (clients) with coaches who have similar lived experiences, they are welcomed into a safe, nonjudgmental space where they can begin to overcome the challenges in their lives related to addiction. Coaches meet members at any point in their journeys. They offer guidance in a personalized way. Members don鈥檛 have to follow a one-size-fits-all program or remain sober to stay engaged with a FIT coach.

Capital Area Counseling Services

Capital Area Counseling Service has been providing services since 1968. They are accredited by the State of South 啵啵直播秀 as a mental health, addiction treatment, and child welfare agency. They serve youth, families, adults and seniors. The agency employs over ninety individuals who provide services in their offices or in the community, such as at schools throughout the area. They provide a variety of SUD services through their highly trained staff. CACS serves the nine counties of Hughes, Stanley, Haakon, Jones, Lyman, Buffalo, Hyde, Sully and Potter. CACS鈥檚 mission is: 鈥淧roviding exceptional and effective care to individuals and families in central South 啵啵直播秀 by empowering those we serve, focusing on strengths, and promoting wellness within our communities.鈥

Additionally, they offer outpatient mental health, addiction treatment, and counseling services including comprehensive assessment services, individualized treatment planning, supportive therapy, intervention groups and relapse prevention to help patients achieve the best results for their unique situation. Coteau des Prairies Health Care System

Coteau des Prairies provides a wide range of health care services to the Sisseton, SD community and surrounding area in northeastern South 啵啵直播秀. Through the Compass Care program, they provide medical, social and mental health care including SUD treatment, Medication-Assisted Treatment and care coordination. CDP鈥檚 mission: 鈥渨e are passionate about the work we do. We believe in offering our patients individualized attention and care, emphasizing their unique needs and treating them as individuals on a human level. We have been successful in this goal, with patient satisfaction scores ranking in the top 3% nationwide! Simply put, our mission is to provide: Quality Care with a Personal Touch, Close to Home.鈥

The Brookings Health System includes a 49-bed hospital, a 79-bed nursing home, and congregate living apartments for seniors. It has consistently been named as a Top 20 Rural Community Hospital in the nation by National Rural Health Association. The Brookings Health System serves the needs of Brookings and surrounding communities, including Arlington, Badger, Hetland, Lake Benton, Lake Preston, Sinai, Nunda, Rutland, Estelline, Dempster, Bruce, Toronto, Astoria, White, Bushnell, Aurora, Elkton, Ward and Volga. Madison Regional Health System 

The Madison Regional Health System is a 22-bed non-profit facility, which was ranked in the top 100 Critical Access Hospitals in the nation in 2022. The Madison Regional Health System has served the Madison and Lake County region for over 135 years, and 鈥減rovides healthcare services to residents who would otherwise have to travel long distances for care.鈥 Its vision is 鈥渢hat Madison Regional Health System serves as a community health focal point through the provision of compassionate, quality, and innovative care鈥 and its mission is 鈥渢o provide quality healthcare, close to home.鈥


The Northern Plains Health Network

The Northern Plains Health Network (NPHN) is a group of independent, medically underserved hospitals that consist of: Brookings Health System (BHS), Huron Regional Medical Center (HRMC), Madison Regional Health System (MRHS), Sisseton Coteau Health Care System Hospital (SCHCSH), and Prairie Lakes Hospital (PLH), and also affiliates with nursing homes, physician clinics, home health services, hospice, and many other local healthcare providers. The five hospitals are independently operated by their own local community board. Three of the five are designated critical access hospitals and the other two are sole community hospitals. The BREATHE-SD project focuses on implementing actionable items at HRMC, BHS, and MRHS. The NPHN commits to helping establish rural healthcare education, workforce development opportunities, and hiring graduates in the following areas: respiratory care, public health, community healthcare workers, case management and para-medicine as needed.

Sioux Falls Health Department

The Sioux Falls Health Department is engaged in the 2022-2025 Community Health Improvement Plan, which looks to improve the health of South 啵啵直播秀ns in response to a year-long collaborative community health assessment. Through the assessment, five priority areas were determined: Active Living, Healthy Eating, Mental Health and Substance Use, Oral Health, and Preventative Care. The Sioux Falls Health Department is partnering with CPIC for two priority areas: Mental health and Substance Use and Preventative Care.

Lost and Found

Lost and Found works to prevent suicide in South 啵啵直播秀 through three distinct areas: Student Programs, Evaluation and Research Services, and Education and Advocacy. Their mission is to "deliver comprehensive, data-driven, and resilience-focused suicide prevention and postvention programs and services for youth and young adults (ages 10-34) and their support networks.鈥" Their vision is: "We envision a world in which no young adult dies by suicide. We also envision a world where tools and support for developing lifelong wellness are easily accessible."
